How to use
- Write the name, type, birth date, sex, and registration number.
- Write size and features, personality, illness or injury, and favorite food.
- In the MEMO grid, write vet and vaccine dates and care duties.

You'll notice changes in weight and health more easily.

Weigh once a month at the same time. Note things before and after odd behavior.
How to use it on our site
- Open the top page and choose the "Notebook" tab
- Pick the Pet Notebook design
- Press "Sample" to see an example entry and how to use it
- Press "Print" to print one sheet free and try it
